Our services will be available to patients who meet the following criteria:

has a life limiting illness and prognosis
-
requires symptom management
-
is aware of and agrees to palliative care referral
-
agrees to the primary goals to control symptoms, offer psychosocial support, maintain quality of life and respect the patients/families EOL wishes
-
must live within Palliative Care At Home’s catchment area
-
must be Covid-19 negative
Referrals for our service can be made by a healthcare professional, or directly from patient, or from their careers, and the following supporting documents may be requested:
-
relevant specialist letters
-
doctor’s notes
-
current medication list
-
medical imaging
-
pathology results
If the referral meets the Palliative Care at Home criteria, our Clinical Nurse Consultant will contact the patient directly to arrange the first admission visit in the home. Our Clinical Specialist Nurse will then contact you to discuss the patient’s plan of care. The plan of care will include obtaining from you:

standard medication order
-
verification of end of life documentation [if available]

Palliative Care At Home Specialist Team are available to provide palliative support and advice to other Health Care Professionals.
Palliative Care At Home endeavours to work closely with the patient's health care team to ensure integrated and supportive care is provided to our patients and their families.
